I understand and agree to the following: *
If this is my graduating semester my CPT end date cannot exceed the last day of the semester.
I must maintain continuous enrollment in a course that requires CPT during the entire semester for which CPT is authorized. Failure to maintain enrollment in the CPT designated course will result in the termination of the student's CPT work authorization and the student will no longer be permitted to legally work.
CPT must be renewed each semester.
CPT grants me work authorization only for the specified dates on my I-20.
I am permitted to only work at the employer listed on my I-20.
Accumulating more than 12 months of full-time CPT will make me ineligible for OPT.
To work only full-time or part-time depending on my CPT authorization (full-time is more than 20 hours per week and part-time is 20 or less hours per week).
I must pick up my new I-20 at ISSS or purchase a shipping label at https://study.eshipglobal.com/ so ISSS can mail me my CPT I-20.
If this is my last semester (fall and spring only) and I will be less than full-time I must submit a Reduced Course Load. (not required for summer)
