The Change of Personal Data Form is used by students in F-1 status, including those on medical leave, to report changes of personal data. Changes must be reported to two offices on campus, the International Office and the Registrar’s Office. The procedures for reporting to the two offices are NOT the same as explained below.
Reporting to the International Office to meet Department of Homeland Security (DHS) SEVIS requirements – For proper maintenance of you DHS/SEVIS record, all students must report changes of location address and home country address to the International Office within ten days of the change, and we in turn report to DHS through SEVIS according to DHS regulations. You must report an accurate location address where you are actually located (not a post office box number). Upon receipt of the update, we update our own office records, and in accordance with DHS regulations, we report those changes in SEVIS, thus relieving you of any obligation to report directly to DHS.

On our form, we also request that you provide various phone numbers and alternate email address such as Hotmail, Yahoo, Gmail, etc. This additional information is NOT reported in SEVIS. It is used only by our office in case we need to contact you.
Reporting to the Registrar’s Office (CatsWeb) – For proper routing of your university mail such as grades and bills, all students must report address changes to the Registrar’s Office through CatsWeb. CatsWeb uses the terms “temporary address” and “permanent address.” They do not have a specific place for you to report your home country address as the International Office does, and we recommend you do not put your home country address anywhere in CatsWeb. We recommend you use your United States address in BOTH the temporary and permanent address fields in CatsWeb because your university mail is sent to your CatsWeb permanent address.
